Clear All

The Clear All button allows you to clear the answers you have selected in the section that you are currently completing. For example, if you are in Section 2, and select Clear All, Section 2 will be refreshed, but all answers provided in other sections of the Tool will remain saved.

Start Over

The Start Over button will clear and refresh all sections within the Tool, and will take you back to the beginning to restart your response.

Save Progress

The Save Progress button will save the answers you have selected in the screen you are in, and all sections prior to that. Every time you select ‘Next’ to move to a new section, your answers will be automatically saved on your device. It’s a good idea to Save Progress if you are going to step away for a short period of time before continuing to complete the Tool.

Login

By creating an account and logging into the form, you are able to save your form permanently. This allows us to save a backup version of your form and for you to login at a future time. All data is stored in accordance with applicable privacy legislation.

Who may support you to complete the SDA Eligibility Tool?

It is your choice! You may wish to complete the SDA Eligibility Tool on your own, or with the assistance of a support person. An example of a support person may include a family member or friend, support coordinator, case manager, clinical specialist, or support worker.
